Medication Reconciliation Initiatives

JCAHO National Patient Safety Goal #8 states that by January 1st, 2006, hospitals must “accurately and completely reconcile medications across the continuum of care”.
 
MSHP Wants to Help:

To help with implementation of this goal, MSHP is:

  • Organizing a conference call for interested parties to discuss and share their progress on medication reconciliation.
  • Providing these sample medication reconciliation forms, ”how-to” guides, and flow charts from the following links.
